We will be collecting forms and payments at the beginning of the meeting. The following are due on Sunday:
- Final payment of balance due (if any).
- Gloria Dei Student Consent and Release Form (if you have not already turned it in).
- Team Effort Permission Slip & Release of Liability - NOTARIZED (if you have not already turned it in).
- Covenant, signed by both the student and parent (if you have not already turned it in).
- $5 per person for lunch.
You MUST turn-in your final payment and forms on Sunday. If you will not be at the meeting on Sunday, you MUST make arrangements to turn-in your final payment and forms to Linda Wild before Sunday (do not turn them in to church - they seem to "get lost in the shuffle" and never find their way to Linda!).
